   Proposal Description:
   Royall et al have proposed a latent dementia construct, known as ???d,??? which is meant to represent the ???cognitive correlates of functional status.??? The d variable has shown promise as a measure of dementia severity in both cross-sectional and longitudinal applications. The current proposal seeks to determine the pathological variables that influence longitudinal changes in d prior to death. We also seek to determine how APOE genotype and cognitive reserve moderate the effect of different neuropathological processes on cognitive change over time. The specific aims of this project are to develop a latent growth curve model for d during life and determine how change in d is influenced by autopsy-verified pathological markers of Alzheimer???s disease, cerebrovascular disease, Lewy body pathology, and frontotemporal lobar            degeneration. We seek to determine the relative contributions of these four pathologies to dementia severity and to determine whether the APOE e4 allele and education (as a proxy for cognitive reserve) moderate the influence of these pathological variables.
